Reporting Instructions

1. Review your Grant Contract for an outline of the information needed.
   If you are unable to find your copy, one is available in your online account.
   

2. Log into the Foundation’s online grant portal using the button in the upper right of this screen.
   

3. Report forms are listed on your home screen. Click the Start button on the left side under Assigned to You.
